Job Description

The primary role of the Reentry Case Manager is to provide direct services that address the employment, vocational, and training needs of offenders/formerly incarcerated individuals and develop an individual employment strategy or employment plan for each offender. The Reentry Case Manager will initiate and maintain ongoing contact with NC Works, local chambers, the 9 th Judicial District Accountability and Recovery Courts, and other partners to develop a pool of potential candidates to assist local companies with workforce needs. The Reentry Case Manager will assist clients in making a successful transition back into the community by providing service coordination, support, and guidance, as well as working with the client to create a plan of action, monitor their progress, and follow up until completion of the plan. The duties of the Reentry Case Manager include, but are not limited to: Assist participants in transitioning back into the community by providing service coordination, support, and guidance Assist participants in creating a plan of action, monitoring their progress, and following up until the completion of the plan Link and schedule appointments with community providers Cultivate a pool of employability training opportunities, job-seeking resources, and job coaching assistance for individuals with criminal histories Develop and maintain a list of employers who are willing to hire justice-involved individuals Input/track all participant data and services by utilizing case management and data collection system
