- to support our tenants in achieving long-term stability.
We are seeking a dedicated Case Manager to provide person-centered, trauma-informed support to tenants facing housing challenges. This role involves conducting comprehensive assessments, developing personalized care plans, connecting tenants with local resources to empower them toward self-sufficiency, and ensuring program adherence through meticulous documentation. The Case Manager will work collaboratively with CFC teams and community organizations to connect clients to essential resources. Essential Function/Responsibilities Work with client caseload to develop and/or strengthen their personal well-being plan according to individual goals and objectives to obtain better housing or maintain existing housing. Maintain contact with all clients on a scheduled basis, appropriate to their particular status and needs, including in their homes and in the community. Support clients through individually tailored services and by facilitating effective connections to community services and resources. Examples of support services include successful tenancy problem-solving, independent living skill-building, connection to public benefits and employment assistance, money management support, and connection to medical, mental health, and substance use disorder services. Assist with income verification for each household for CFC Rental Assistance and CFC records. Facilitate move-out/move-in process for tenants to other CFC properties or alternative, appropriate affordable housing. Be an effective mediator and advocate for our clients when needed. Maintain accurate records, case notes, and case documentation to ensure compliance with program goals and reporting standards. Collaborate with internal teams and external partners, and attend regular team and case management meetings, supervision, and training as required. Communicate effectively in oral and written forms with clients and their families, as well in the workplace with co-workers, supervisors, other service agencies, and the community. Build professional relationships with clients, service/resource providers, and community partners. Work closely with the Lead Case Manger to align case management strategies with program objectives and ensure service delivery consistency. Other responsibilities as assigned. Qualifications Bachelor’s degree in Social Work, Human Services, or related fields and 2+ years of experience in case management, housing services, or social work. Or 5 years of experience in case management, social work and housing services. Proficiency in case management processes and documentation. Familiar with local, state, and federal housing resources and policies. Exceptional professionalism, time management, communication skills, and team collaboration capability. Experience with Slack, Zoom, Google Drive, Sales Force, and Microsoft Office.
