P.A.T.H. (Project for Assistance in Transition from Homelessness) The position of Peer Support Specialist•Case Manager (PSSCM) is an integral part of the Coastal Horizons PATH Program team approach to integrated care for behavioral health and medical care, providing direct services to the most vulnerable, underserved adults experiencing chronic homelessness. The
PATH PSSCM
utilizes their own history and lived experience to assist those referred to the PATH program to remove barriers to accessing community resources including: treatment for mental health, housing, or other area needed for an individual to gain stability.
PATH PSSCM
conducts outreach to adults living in outside locations such as streets, wooded areas, abandoned buildings or under bridges.
PATH PSSCM
makes connections with EMS, local shelters, community partners, to develop a relationship to further advance the program. Qualifications /
Skills:
Personal history of lived homelessness and as consumer of behavioral health services preferred, or experience/demonstrated knowledge of care and wellness plan coordination, in addition to housing resources or homelessness. One (1) year of recovery with mental health or substance use required. PSS Certification/WRAP Preferred.
