Job Description:
The Supportive Housing Program (SHP) Case Manager for First Fruit Ministries is responsible for comprehensive case management in the ministry’s housing programs for victims of human trafficking. The purpose of this position is to bridge the gap between homelessness and housing placement. The person in this position will follow the process of a client as they progressively increase toward their identified goals. The SHP Case Manager will support clients in removing barriers and navigating community resources and referrals. This position not only requires excellent case management skills that include identifying goals, fostering therapeutic progress, increasing income, and gaining safe housing, but also intense determination. This position requires the accompaniment of one client throughout their entire journey to housing, providing the continuum of care that so many clients need. The program’s holistic approach aims to address the obstacles and unmet needs that keep individuals and families in dysfunctional cycles of homelessness or precarious housing placements. The SHP Case Manager will navigate the nuanced work of discerning the condition of people in crisis and their eagerness to move on from limiting or destructive behaviors or thinking. The person should be prepared to provide guidance through stages of healing emotionally, mentally, physically, and financially.
Duties:
Supportive Housing Program Case Manager for First Fruit Ministries’ is responsible for case management of homeless or trafficked individuals as they apply to and participate in The Guest House, 5.11 House, and crisis response housing options including hotel voucher programs. The duties for the Supportive Housing Case Manager consist of, but may not be limited to, the following: Quality Programs Facilitate high performance of the supportive housing program Maintain all client records and program statistics according to the standards of various funders Regularly review program progress reports Data Collection and Analysis Input client and administrative data for reports through NCHMIS, EmpowerDB, and a separately maintained spreadsheet for Supportive Housing Program Director and Data Analyst Review Share monthly reports with the Programs Director Organize data to prepare for requests from Executive Assistant that inform grant management and performance Agency Collaboration Collaborate with the Community Engagement Director to effectively utilize Volunteers, and Advocates in supportive housing programming. Collaborate with the Housing Specialist to assist clients in achieving their goal of permanent housing Collaborate with The Guest House staff to transition clients from the emergency shelter to long-term transitional housing in the 5.11 House program or other appropriate housing options. Request prior approval from Programs Coordinator for program expenditures Collaborate with Operations Manager to create effective systems to execute the work of the ministry Community Relationships Establish and maintain positive, productive working relationships with mental health programs, shelter programs, law enforcement and providers of services and resources to homeless neighbors Create and maintain consistent communication channels, both verbal and written, between several parties (i.e. referral source, collaborating agencies) Develop an effective, timely referral network in order to ensure ongoing direction and support as needed. Case Management/Client Outcomes Seek supervision from Programs Coordinator in collaborating with Supportive Housing team to determine client eligibility per program Meet grant-specific performance measures for multiple funding sources Coordinate with Housing Specialist to find safe housing placements for clients Facilitate the effective transition from the streets to enrollment in 5.11 House, The Guest House or emergency hotel placements Provide supportive services to clients including but not limited to case management, referrals to substance abuse and mental health treatment, referrals to medical providers, and connection to social services Provide advocacy for clients and their pets (when applicable) when they encounter barriers Provision of transportation limited to the confines of approved destinations Assist participants in development of a strength-based/solution-focused individualized goal and action plan that promotes safe housing and self-sufficiency Identify participant strengths and barriers to stability and assist participants in reducing barriers and linking to resources and services. Create and maintain client files that gather and interpret information pertaining to social, personal, environmental, and health information Maintain reports and logs that are consistent with the requirements of numerous grants (i.e. mileage, match, etc.) Contribute to the data collection of performance measures in a timely manner
