Job Description
Case Manager Eastern Carolina Human Services Agency, Inc. Jacksonville, NC 28540 The Case Manager will collect, organize and analyze information from applicants to determine program eligibility and develop a plan based on the participant’s needs and goals and the resource services available. Follow up will be conducted with each enrolled family to ensure cooperation and compliance with agency agreement. Counseling and training may be conducted to enhance achievability of goals. Attendance may be required at workshops or agency training sessions. Client confidentiality must be maintained. This position requires a combination of remote work serving New Hanover County residents and some in-person duties in Onslow County. Candidate must be able to travel between locations as needed.
