Our lifechanging programmatic work includes trauma-focused mental health services, domestic violence interventions, supervised visitation, housing stabilization services, homeless engagement, and financial counseling. Job Summary:

Triangle Family Services seeks a full-time Street Outreach Specialist to join our Homeless Engagement Team to provide street-based assessment, case management, and emergency survival supplies to individuals experiencing unsheltered homelessness. Job duties include but are not limited to the following:

  • Provide outreach and engagement to individuals or families experiencing unsheltered homelessness
  • Conduct initial assessment and assist in the referral process to available shelter and housing programs through Coordinated Entry
  • Conduct thorough assessments of clients’ needs, strengths, and goals
  • Utilize evidence-based practices to engage and motivate clients; advance a housing-focused approach to service planning
  • Assist clients with daily living skills development and provide concrete support for and linkages to basic care as needed
  • Provide timely and appropriate referrals and resources to clients to ensure connection with community supports, programs and benefits
  • Maintain client records and service documentation in compliance with funder requirements
  • Participate in internal and external meetings as needed to ensure services are coordinated with broader service community
Qualifications:
  • Bachelor’s degree in social work or a related field or equivalent experience
  • Prior or current experience with direct service delivery in one or more of the following areas: outreach and engagement work to underserved populations; serving persons/individuals/families experiencing homelessness esp. with significant mental or behavioral health concerns; with one or more housing stabilization interventions
  • Familiarity with prioritization tools and HMIS (Homeless Management Information System) or equivalent
  • Knowledge of local housing inventory and/or an understanding of community agencies and resources that serve at-risk populations
  • Bilingual (English-Spanish) skills a plus The position involves extensive interaction with clients in non-traditional, outdoor work settings, so must have reliable transportation for extensive local travel.

Work schedule may involve occasional evening and/or weekend hours. Home office provisions are provided.
