Role:
This employee works to assure that unsheltered homeless individuals/families gain access to appropriate shelter and other necessary services.
Essential Functions & Responsibilities:
E 15% Identify clients who are unsheltered homeless or at imminent risk of homelessness through direct outreach activities and through reports/referrals from 211, shelters, police, churches, town welfare officers, human service providers and others. E 20% Initiate contact and establish a working interaction with individuals/families who are unsheltered or at imminent risk of homelessness for the purpose of providing assistance with attaining/maintaining adequate shelter and access to needed services. Assess the immediacy of need and type of intervention appropriate for each individual. E 5% Take appropriate action to deal with any homeless emergency situation. Assist homeless people with obtaining transportation to shelter or services. E 10% Establish & maintain positive, productive working relationships with mental health offices and other members of the local homeless services continuum. Serve as a liaison between Mental Health Professional staff and other TCCAP staff dealing with mutual clients and homeless issues. E 5% Develop and assist with implementation of shelter client’s service plan utilizing CTI Case Management. N 5% Assist shelter managers in arranging for alternate shelter for homeless being turned away due to capacity or criteria problems N 10% Participate in appropriate workshops, trainings, meetings and Homeless Services Continuum’s. E 25% Maintain client records, activity logs and complete reports. Enter client information into State HMIS system and EmpowOR. Maintaining confidentiality in all aspects of position. N 5% Other duties as assigned

Knowledge, Skills and Abilities:
Experience Three months to twelve months of similar or related experience. Education (1) A two-year college degree or (2) completion of a specialized course of study at a business or trade school or (3) completion of specialized training courses conducted by equipment vendors or (4) job specific skills acquired through on-the-job training or apprenticeship program. Interpersonal Skills Courtesy, tact, and diplomacy are essential elements of the job.
