The Case Manager•Transitional Housing provides case management to veteran households and their families with creating an Individual Service Plan, aids in finding stable housing, assisting with ing for financial benefits such as

SSI, SSDI, VA

Benefits, Food Stamps and long term supports, and resources that may be of benefit.

Requirements Duties & Responsibilities Maintain a case load of approximately 10-15 active clients
Meet with clients for intensive case management in a housing first model
Maintain daily documentation of interventions and services provided to program participants
Maintain appropriate records of all financial transactions, client documentation, etc.
Perform/prepare for case file audits when necessary
Assist clients with transportation, when necessary
Maintain all necessary documentation for Veterans Affairs
Maintain positive communication with Veteran Affairs, community allies and professional contacts
Complete inactive client documentation when client is discharged from the program
Collaborate with all Harbor Care staff as needed
Continue to acquire professional education in best practices related to veterans and housing
Participate in all Transitional Housing team meetings, webinars, and trainings
Maintain current calendar of all professional activities in Outlook
Support veteran Family members residing with the veteran
Some travel will be required Basic Qualifications Bachelor’s degree in human service field required
Must possess a valid driver’s license
Computer proficiency in MS Office programs, and general internet research and navigation skills
Be of good moral character Preferred Qualifications Master’s degree in Human Services or Social Work
2 years Case Management experience working with vulnerable populations
Background working with/in the U.S. Military Knowledge, Skills & Abilities Self-directed & works well independently
Adept at building relationships and rapport with clients, colleagues and community allies
Excellent clinical boundaries with clients and colleagues
Superior communication skills•verbal and written
Understanding of veteran culture and VA process
Ability to listen & follow directions
A spirit of open-mindedness, compassion and inquiry
Knowledge and expertise in Social Work, Mental Health and/or Human Services
Creativity, strong time management, and trouble-shooting skills
Ability to express or exchange ideas by means of the spoken word
Ability to receive detailed information through oral communication
Ability to perform repetitive or routine duties and decision making
Ability to endure periods of heavy workload or excessive stress over a reasonable length of time
Ability to work with frequent interruptions, and respond appropriately to unexpected situations
Ability to exert up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Primary functions involve sitting, most of the time. Walking or standing required occasionally.
