Case Manager (Full Time)
Department:
Behavioral Health Services (NJCRI-LTBI) Reports to : Program Director
RESPONSIBILITIES
The Case Manager will complete screenings, including GPRA and NJSAMS, for all new admissions to NJCRI’s LTBI to determine risk needs. Will complete 70 per year. The Case Manager will also be responsible for the coordination of other services for the client, such as housing, insurance enrollment assistance, and supportive services. The Case Manager will coordinate and conduct urine drug screening weekly participants participants Provide ongoing case management services to coordinate all aspects of care, including behavioral health, primary care, HIV and hepatitis treatment, housing, insurance enrollment assistance, and supportive services. Conduct initial GPRA interviews and follow-up GPRA interviewers at 6 months time points. Develop individualized service and relapse prevention plans Deliver client services as described in the grant award. Facilitate three groups weekly. Collaborate with other departments to enhance client services. Document intakes, referrals and follow-up data for monthly reports Assists the Project Director in preparing reports. Document all services in accordance with the dictates of individual funding contracts.
Assist with quality assurance including:
administering and collecting client satisfaction surveys and outcomes-based internal evaluations for the Project Director and Project Evaluator. Assist the Director in evaluation and improvement of program services. Interact with other service providers to enhance coordination of services and referrals to the program. Participate in governmental, professional and community committees, task forces, as assigned by the Director and approved by the Chief Executive Director. Foster and preserve a strong team spirit among program staff and volunteers. Enhance one’s professional abilities via continuing education. Attend in-service training, and continuing educational opportunities relating to HIV disease, HIV prevention counseling, substance abuse prevention, case management or other related topics.
JOB QUALIFICATIONS
Education:
Minimum High School Diploma. Good written and oral communication skills.
Experience:
Experience in mental Health and substance abuse.
