You will work with elderly and disabled populations in West Orange, NJ. As a Case Manager, you will earn $45,000 a year and work full-time, Monday to Friday, from 9AM to 5PM. In this role, you will be responsible for assessing client needs initially and on an ongoing basis to identify needed care and services.
This will include:
- Teaching clients money management skills, such as how to develop and maintain budgets, use banking services, and plan for major purchases.
- Completing ledgers, reconciling accounts, and assisting with spending choices to gradually promote clients’ personal finance literacy.
- Establishing a collaborative relationship with involved family members to advance identified goals, provide supportive services, and refer clients to the clinical services division, if more in-depth services are required.
- Assuming leadership in interactions with representatives of the NJ Division of Developmental Disabilities (DDD) or assigned support coordinators.
- Reporting and documenting case notes in a concise, understandable, and legible manner in the electronic health record (EHR) that demonstrate a direct relationship to the achievement of treatment goals as defined in the ISP or the Annual Case Evaluation.
- Completing daily logs, accident and incident reports, outcome data sheets and all appropriate Agency and government forms in a timely manner as well as maintaining client calendars of important medical events, exams, and medication.
- Determining clients’ continued eligibility for entitlements under the guidance of the Director of Operations.
- Visiting clients’ homes at least monthly to ensure a safe, clean, and hygienic environment with sufficient food and clothing. To be considered for this opportunity, you must have at least 3 years of Case Management experience OR 10 years of work experience in a related field, a bachelor’s degree, and have:
- Familiarity with office equipment and basic computer skills, including EHR and Microsoft Office.
- Availability on some weekends and after 5pm, if needed.
- The ability to attend trainings and maintain certifications, as required.
- A valid driver’s license.
- A valid CPR/First Aid Certification.
