The program is designed to provide evidence-informed and effective reentry initiatives for individuals returning from prison, ensuring the supportive service coordination of rehabilitative, academic, and vocational training. The Reentry Case Manager will play a pivotal role in supporting returning citizens through a structured three-tier program, helping them reintegrate into society successfully and achieve independence.

Key Responsibilities:

Tier 1: Pre-Release Assessment Conduct a reentry needs assessment to identify service needs, including counseling, education, employment readiness, and benefits. Develop a personalized Service Plan (SP) based on assessment results, outlining housing needs (emergency, short-term, long-term) and integration goals in line with parole requirements. Tier 2: Residency Phase Work closely with NJLEAD participants residing in Muladhara House. Collaborate with participants to actively pursue goals outlined in their Service Plan. Provide structured support and services to facilitate progress. Tier 3: Full Reentry Assist participants in transitioning to securing sustainable employment and permanent housing. Support participants in achieving independence and maintaining a sense of belonging and purpose. Sustain connections with participants beyond the program’s implementation period to ensure ongoing support and success.

Qualifications:

Bachelor’s degree in Social Work, Psychology, Criminal Justice, or a related field. Minimum of 2 years of experience in case management, reentry services, or related fields. Strong understanding of the challenges faced by formerly incarcerated individuals. Excellent communication, organizational, and interpersonal skills. Ability to work independently and as part of a team. Commitment to the mission of PHF and the NJLEAD program.

Preferred Qualifications:

Master’s degree in Social Work or a related field. Experience with evidence-based practices in reentry and rehabilitation. Familiarity with local resources and services for returning citizens.
