- Help individuals navigate services, access opportunities, and receive the support they need . Develop Personalized Support Plans
- Conduct face-to-face home visits and ensure safety, health, and well-being . Provide Crisis Support
- Step in when challenges arise, ensuring rights are protected and emergencies are handled with care. Track Progress & Stay Organized
- Maintain detailed documentation and submit timely reports . What You Bring Bilingual (Spanish/English)
