With PATH services, our caring staff provide case management, assistance with linkage to community mental health and housing services, and education and skill development related to accessing services and benefits.

Hourly Responsibilities:

Identify consumer housing needs and existing barriers to permanent housing Provide ongoing assessment of consumer’s emotional well being, clinical symptomology, behavioral changes and medication compliance Engage consumers in treatment plan creation Provide direct care as delineated in the individualized CTP Provide advocacy and linkage services to prevent homelessness Assist consumers with ADLs Provide assistance to consumers with obtaining basic needs Requirements High School Diploma or Equivalency Two years experience working with adults with a mental illness. Experience with homeless programs preferred.
