As the Harm Reduction Case Manager , you will play a pivotal role in individuals’ lives, making a significant impact through: Review assessments and discharge plans provided by medical staff, CASAC, or other facility staff Utilize assessments and discharge planning provided by correctional facility partner to develop a plan of care Provide Opioid Overdose Prevention training in accordance with agency guidelines Provide Enrollment into the agencies Syringe Program Provide HIV and HCV Screening as necessary

$42,000

Harm Reduction Case Manager Maximum Salary:

$42,000

Harm Reduction Case Manager Schedule:

Mon – Fri 8:30am – 4:30pm

Qualifications:
