Case Manager (Health) Chautauqua County Government Mayville, NY 14757 Applications accepted at https://chautauqua-portal.mycivilservice.com/ The Chautauqua County Department of Health & Human Services is seeking a compassionate and detail-oriented Case Manager to support our Early Intervention Program. This full-time position is based in our Mayville, NY office and involves a combination of field and office work to support families with infants and toddlers who have developmental delays or disabilities. As a Case Manager, you will play a critical role in helping families navigate services, coordinate care, and ensure that children receive the support they need to thrive.
Minimum Qualifications:
Either A. Graduation from a regionally accredited or New York State registered four-year college or university with a Bachelor’s degree in a Health or Human Services related field; OR B. Successful completion of 60 semester credit hours from a regionally accredited or New York State registered college or university in a Health or Human Services related field and two (2) years of full-time, paid experience providing case management services to young children and their families; OR C. Four (4) years of full-time, paid experience providing case management services to young children and their families.
Addition Requirements:
A. Ability to meet the regular transportation requirements in carrying out fieldwork assignments at time of appointment and during service in this classification. B. Candidates must not have been convicted of an alcohol or drug related driving offense, or have been convicted of refusing to submit to a Blood Alcohol Content (BAC) or chemical test for a period of three (3) years prior to the date of written test. Additionally, candidates that pass the written test and appear on the eligible list must not have been convicted of an alcohol or drug related driving offense, or have been convicted of refusing to submit to a Blood Alcohol Content (BAC) or chemical test for a period of three (3) years prior to certification and appointment.
Key Responsibilities:
Provide case management services through home visits, phone calls, and collaboration with medical, social service, and community agencies. Facilitate the development and ongoing review of Individualized Family Service Plans (IFSPs) in partnership with families and providers. Conduct intake assessments and refer families to appropriate services. Schedule and coordinate developmental screenings and evaluations. Educate families on their rights and available advocacy services under the Early Intervention Program. Maintain accurate documentation and submit reports as required by county, state, and federal regulations. Participate in trainings, meetings, and continuing education to stay current with best practices. Perform site visits at homes, work sites, and other community locations.
Qualifications:
Strong communication, organization, and interpersonal skills. Ability to work independently and collaboratively with families and service providers. Experience in human services, healthcare, or related fields preferred.
Additional Information:
This position is being filled on a provisional basis, pending the results of an upcoming Chautauqua County Civil Service Examination for the Case Manager (Health) title.
