Job Description:
Changed by Christ is a Christ-centered reentry and recovery organization in Clinton County, New York. We are seeking compassionate and detail-oriented individuals to serve as Social Needs Screeners. This vital role focuses on identifying the health-related social needs (HRSNs) of our community members. Screeners help connect participants to critical services such as housing, food, employment support, and spiritual care.
Key Responsibilities:
Conduct Intake Screenings:
Use standardized tools to assess needs related to housing, food security, transportation, employment, education, interpersonal safety, and recovery support.
Assess Eligibility:
Determine eligibility for empowerment services, transitional housing, peer support, and referrals to community-based services.
Coordinate Care:
Work closely with Peer Support Specialists, Care Managers, and House Managers to assign participants and ensure continuity of care support.
Manage Referrals and Follow-Up:
Enter and manage referrals in our internal tracking system, ensuring timely follow-up and documentation of progress. Engage with
Compassion:
Build trust and maintain confidentiality while providing a trauma-informed, nonjudgmental, presence during screenings.
Document and Report:
Maintain accurate, up-to-date records of screenings, referrals, and follow-up outcomes; participate in team meetings and service planning.
Qualifications:
Faith-Based Commitment:
A personal commitment to the mission and values of Changed by Christ, and an ability to uphold a positive perspective in all interactions.
Educational or Experiential Background:
Experience in peer support, social work, case management, or similar fields is preferred. Lived experience is valued.
Strong Interpersonal Skills:
Able to engage sensitively with individuals facing trauma, addiction, homelessness, or reentry challenges.
Organizational and Technological Skills:
Ability to manage multiple referrals, enter documentation into systems, and maintain confidentiality.
Cultural Competence:
Demonstrates respect for diverse backgrounds
Job Type:
Full-time Pay:
$18.00 – $20.00 per hour Expected hours: 40 per week
