Full-time Supportive Housing Case Manager Shift/schedule:
Monday-Friday, 7:00am-3:30pm
Pay Range:
$17.50/hour to $22.00/hour
Position Summary:
Responsible for fulfilling SJATRC mission by accomplishing organizational goals, work with participants to help them learn how to maintain stable housing and advocate in their best interests. Assist Housing program participants by continually assessing strengths and needs, linking participants to natural support systems, identifying existing community resources, coordinating person-centered, individualized Service Plans, and fostering self-sufficiency.
Education and Training:
High School Diploma or equivalency certificate.
Essential Duties:
+ Administers the Supportive Living Facility assuring all decisions are based on the scope of the mission, the philosophy of SJRC and the spirit of the existing policies and practices. + Works closely with community resources and human service providers to ensure that all options are made available to tenants. + When necessary, acts as advocate on behalf of tenants to assist them in obtaining necessary and needed entitlements. + Assist in Monitoring progress of each tenant at all 4 housing and documents progress/regression in tenant files. Meets with tenants individually as needed. + Insures the provision of safe and secure residential environment for the safety and personal growth of the tenants. + Facilitates tenant meetings and records meeting minutes. + Maintains a cooperative and productive working relationship with other agencies and represents SJRC in a professional manner. + Fosters the mission of SJRC through commitment to quality, facilitating and coordinating effective working relationships within the organization and with all externally who have contact with SJRC, and demonstrating respect and dignity to clients and staff. + Is trustworthy in safeguarding confidential information. + Adheres to SJRC’s Code of Ethics, Confidentiality and all applicable policies and procedures. + Ability to handle sensitive and challenging situations with tact and discretion. + Ability to communicate effectively verbally and in writing. + Ability to be supportive in a team effort. + Maintains current NYS license(s) and/or credentials as appropriate. + Oversight of proper maintenance/cleaning of the apartment unit. + Assisting housing participants with budgeting, menu planning and purchasing nutritious foods, and when indicated developing cooking skills. + Emphasizing the importance of keeping medical and clinic appointments, taking prescribed medications as indicated by their physician, as well as, encouraging exercise, self-care, and positive use of free time. + Conducting pre-screening & intake assessments. + Developing Service Plans for all housing tenants with updates on a quarterly basis. + Maintaining resident charts/records, and documentation as required. + When indicated, completing case closing documentation and discharge planning. + Collaborate with DSS in the Family Unity Program to track, process, and submit all vouchers for payment, ensuring accuracy and timely submission. Maintain organized records by filing all documentation accordingly. + Review all previous shift notes to identify concerns, promptly notify the supervisor of any issues, and ensure proper documentation and daily filing. + Conduct monthly home visits to each housing location, providing ongoing support and case management to ensure resident stability and well-being. + Track and monitor daily shelter inspections and assist the Associate Director of Housing with monthly housing inspections across all housing programs, ensuring compliance with standards, maintaining proper documentation, and filing records accordingly. + Ensure all staff complete contact notes in a timely manner and document them in tenants’ charts to meet regulatory requirements.
