Case Manager – Supportive Housing – Local Contract 4.2 4.2 out of 5 stars Brooklyn, NY Our client, a large social service nonprofit organization, is seeking a Case Manager for Supportive Housing in Brooklyn, NY. This role offers the opportunity to provide vital support and guidance to young adults. Job Function:
Day Shift. 5×8: 9am – 5pm. Collaborate with residents to develop client-centered, recovery-oriented service plans. Use a harm reduction approach to support individuals with mental health and substance use challenges. Coordinate care by making referrals and liaising with health, mental health, substance use, and employment service providers. Engage residents through regular home visits, community outings, and appointment escorts as needed.
Job Requirements:
Experience with ETO or other electronic case management systems. Knowledge of evidence-based models, particularly Motivational Interviewing, preferred. 1 year of experience working with single adults facing homelessness, mental illness, and substance abuse issues. BS/BA in Social Work, Sociology, Psychology, or a related field, preferred.
