Program Director Job Summary:
The Homeless Street Outreach Lead Case Manager works both in the office and in the field across Niagara County, overseeing outreach efforts, leading a team, and providing direct client support. This role manages outreach schedules, ensures trauma-informed practices, and coordinates with case managers and the Program Director. In addition to leadership responsibilities, the Lead Case Manager maintains a small caseload, connecting clients to shelters, housing programs, and supportive services. Focusing on homeless domestic violence victims, while remaining flexible to address the needs of the broader homeless population engaging Heart, Love and Soul (HLS) as necessary.
ESSENTIAL DUTIES
Engagement and Service Provision
- Leads and coordinates outreach activities, scheduling regular visits to engage unsheltered individuals across Niagara County, including rural and underserved areas.
- Directs outreach efforts through regular communication, meetings, and collaboration with case managers and the Program Director.
- Conducts street outreach and engagement with unsheltered individuals at locations such as soup kitchens, shelters, encampments, and public spaces.
- Provides immediate support to clients, including referrals to medical, mental health, and substance abuse services.
- Assists clients in obtaining emergency shelter, permanent housing, and eligible Federal, State, and local benefits (e.g., SSI, SNAP, TA, veterans’ benefits).
- Utilizes SOAR (SSI/SSDI Outreach, Access, and Recovery) to assist individuals experiencing or at risk of homelessness in applying for Social Security Disability benefits, particularly those with serious mental or physical health conditions.
- Conducts intake and housing-related assessments, supporting complex cases, including domestic violence survivors, individuals with substance use disorders, and those with serious mental health conditions, by developing comprehensive service plans that address both immediate needs and long-term goals, such as housing stability and behavioral health care, while remaining flexible to serve the broader homeless population engaging with HLS.
- Facilitates crisis interventions, referrals, and hospitalizations as appropriate.
- Obtains historical and collateral information from multiple sources to support participants’ behavioral and physical health needs.
- Continuously assesses participant needs using conversational and observational methods as well as formalized risk assessment tools, especially for individuals identified as high-risk.
- Conducts intake and housing-related assessments, focusing on homeless domestic violence victims, while remaining flexible to address the needs of the broader homeless population engaging HLS as necessary.
- Maintains regular contact with clients, providing case management and follow-up to ensure ongoing support and service evaluation.
- Coordinates in-reach and outreach efforts to maximize service delivery and connections to resources.
- Participates in weekend, night, and/or early morning outreach with the outreach team as needed.
Record Keeping Maintains accurate records of guest interactions in assigned data systems, including the Homeless Management Information System (HMIS). Prepares reports and documentation in a timely manner. Communicates relevant guest information to staff while adhering to confidentiality and HIPAA regulations. Other Attends and participates in staff and community meetings, and training sessions including the Niagara County Coalition for Services to the Homeless. Participates in agency-wide special events such as holiday programming, fundraising events, etc. as requested. Performs additional duties as requested, including occasional evening or weekend responsibilities.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
Familiarity with the human services network in Niagara Falls or similar communities. Strong verbal and written communication skills with attention to detail. Proficiency in computer systems and preferred experience with databases. Abilityto utilize problem-solving skills to effectively guide and support team members, fostering collaboration, and accountability, while ensuring alignment with organizational goals.
- Ability to prioritize tasks effectively and maintain a professional demeanor in high-pressure situations.
QUALIFICATION STANDARDS
Master of Social Work Degree (MSW) required. Experience in social work, case management, or community outreach. Crisis intervention training and knowledge of trauma-informed care practices preferred.
ERGONOMIC REQUIREMENTS
This role requires the ability to sit, stand, stoop, bend, lift up to 50 pounds, carry items, and perform some repetitive movements. All ergonomic requirements must be performed without significant risk of injury to oneself or others or with reasonable accommodations to ensure the essential functions can be carried out safely. We have developed policies and procedures to promote the safety of all staff.
