PM Job Summary:

FLACRA is seeking a dedicated andcompassionate Reentry Case Manager to support individualstransitioning from incarceration at Ontario County Jail back intothe community. This role is vital in connecting individuals tohousing, treatment, employment, education, and care managementservices. The Reentry Case Manager will also ensure that jailcontract obligations are fulfilled, services are effectivelycoordinated, and progress is tracked and reported.

RequiredQualifications:

Associate’s Degree in Chemical Dependency, CriminalJustice, Psychology, Counseling, or a closely related field

ValidNew York State Driver’s License Preferred Qualifications:

QualifiedHealth Professional (QHP) status CASAC certification or Bachelor’sDegree in Health or Human Services Familiarity with jail-basedservices and community reentry programming

Skills and Experience:

Excellent verbal and written communication skills Demonstratedability to work with diverse populations and service providersStrong knowledge of local behavioral health systems and communityresources Ability to multi-task, manage time effectively, andremain calm in crisis Proficiency in documentation and use ofelectronic health records (EHR) Strong interpersonal skills andability to work independently and as part of a team
