Woodhaven Blvd, Rego Park, NY 11374 Assisted living facility has an immediate opening for a Case Manager position. Queens Adult Care facility looking to fill an entry level case management position. The qualified individual will have an Associates degree or higher with a concentration in Human Services.

Required skills:
