Under the broader mission of eliminating racism and empowering women, our focus is to strengthen and empower the community through educational opportunities, skills, and resource attainment in our main program areas: Pre-GED and GED, Adult Literacy, After-school, Social Services and Senior Programs richly infused with arts, culture & wellness. With 3 sites in Flushing and Jamaica, most of our members, students & clients are immigrants who need culturally accommodating and language appropriate services.

EXPECTATIONS

Reporting to the Director of Senior Services, the position is an excellent opportunity to serve the diverse seniors (60 & over) of the community, in meaningful ways. The Case Manager (Family Development Coach) will be stationed in Flushing Queens, and may work across other anchor institutions in the community. She/He will assist and coach seniors and their families/care givers in the two major areas of our Programs with a strong emphasis on client self-advocacy. , with accessing vital social services in the community, including emotional health services & referrals, free phones, reduced fare metro cards, SNAP (food stamps), food pantry, housing applications & rental assistance, etc. identifying & registering clients in our robust wellness, exercise, & arts/education classes such as yoga, dance, strength & balance, ESL, art, health classes, trips, etc. to meet their health, fitness, creative and social engagement needs The Case Manager/Coach will meet daily with clients to identify eligibility and specific needs through an intake process, creating, tailoring, and documenting a service plan to help clients achieve specific goals. The successful candidate is passionate about working with seniors and will conduct telephone and community outreach to identify new clients working collaboratively with other agency staff. The Case Manager/Coach will ensure that he/she develops strong partnerships with local schools and other community-based organizations, to ensure the YWCAQ effectively deploys its wrap around services to Flushing and surrounding communities.

Role Responsibilities:

Provide one-on-one case management/coaching with Mandarin and English-speaking senior clients and families to understand their needs and eligibility for participation in our program. Conduct a detailed intake session & develop Individual Support Plans (ISP) outlining the goals of the client Apply for identified resources/services needed through multiple social services databases and/or enrolling clients in wellness, exercise & arts/education classes Provide effective solutions across program services, take steps to achieve measurable goals and deadlines within specified timeframes and continuously track progress Oversee each client through regular communication, coaching and formal documentation of interaction/case notes in our DYCD database and excel worksheets Assist in planning and conducting workshops, special events senior advisory council meetings Expand our current portfolio of partnerships with community resource agencies and not-for-profits and help to develop strategies to effectively provide case management services. Plan/attend community outreach events, tabling, flyering, etc. and attend Partner and/or Funder Meetings representing YWCAQ in a highly professional way. Support YWCA of Queens, its other programs and attend weekly staff meetings as requested. Implement continuous improvement mechanisms to ensure program innovation and high-quality services. Meet weekly with the Director of Senior Services to discuss program outcomes, challenges, upcoming events and/or initiatives to ensure compliance of grant agreement and advance program expansion. Occasional weekend/evening work will be required.

Qualifications:

Fluency in Mandarin Chinese & English Required Associate’s Degree required; Bachelor’s preferred Experience and desire to work with Seniors and Immigrant populations preferred At least 1 year of relevant case management experience that includes demonstrated cultural competency & sensitivity Superior time management, interpersonal, writing, organizational skills, and a keen attention to detail Proficient in

DYCD & HRA

databases, MS (Word/Excel/Outlook/etc.) and Google Docs/Sheets preferred Strong strategic, analytical, communication, and interpersonal skills Willingness and initiative to roll up one’s sleeves, problem solve; learn new systems and processes, approaching challenges with a “can-do” attitude and a sense of humor Ability and willingness to work collaboratively with other program staff, and other agencies Integrity, credibility, and a commitment to and passion for the YWCA of Queens’ mission.
