Service Coordinator At Home Advocacy, Inc. 12 Spring Street, Schuylerville, NY 12871 Responsible for

NHTD/TBI

Medicaid Waiver caseload. Duties will include facilitating team meetings, case management, writing service plans, connecting clients with community resources and assist them with obtaining services, monitoring client’s health, safety and their satisfaction with the Waiver Services. Must be self-directing and detail oriented, with the ability to work independently as well as collaborate effectively with patients and other health care professionals. Will be working with clients who are elderly, disabled, sick, or with a Traumatic Brain Injury. Must have Master’s Degree in related field with (one) year of experience in case management or a Bachelors Degree with (three) years of case management or an Associate’s Degree in related field with (five) years of case management. Paid training provided. Must enjoy working with people and be committed to improving their quality of life that fit their individual health care needs. Possible opportunity to perform some job duties remotely.
