Case Manager Location:

Abraxas WorkBridge, Pittsburgh, PA 15222 Abraxas WorkBridge is hiring a Case Manager to provide opportunities to youth involved with Allegheny County Juvenile Probation to complete court ordered community service hours through an established community service site. The Case Manager maintains a client case load from intake to discharge. Responsibilities include finding an appropriate nonprofit community service site to complete court ordered community service hours, conducting placement interviews with site supervisor and youth/family, and establishing a schedule conducive for youth to be successful in completing the hours. The Case Manager is responsible for monitoring each youth’s hours through follow up and updates from each community service site and reporting progress to the referring Juvenile Probation Officer (JPO). Once obligations are met, the Case Manager is responsible for case closure and submission of discharge paperwork. This position requires travel throughout Allegheny County.

Case Manager Hiring Requirements:

High school diploma and minimum of three years of related experience in the juvenile justice system; OR Bachelor’s degree in related field preferred and minimum of one year of related experience in the juvenile justice system. At least twenty-one (21) years of age Driver’s License Physical exam that includes TB and drug testing. Criminal clearances (State Police, FBI and State Child Abuse Clearances) Satisfactory completion of background screening and applicable pre-employment checks; including but not limited to employment and/or personal reference and driving records. Ability to work with computers and the necessary software typically used by the department. Must pass both federal and state background checks which screen for crimes including, but not limited to drug related crimes, violent crimes, sexual abuse, domestic violence, child abuse, and repeated misdemeanors

Case Manager Basic Responsibilities:

Maintain a client caseload with appropriate service plans. Develop and maintain relationships with community service sites. Develop and maintain relationships with assigned Juvenile Probation Department. Orient the client to program norms, rules, and behavioral standards. Collect the YLS assessment from the Juvenile Probation Department and implement it into the service plan and the service delivery. Build and maintain a positive relationship with the youth and their family through being caring, consistent and competent. Support clients and their families in completing any court ordered conditions including restitution, community service, or specific counseling needs and assessment. Supervise client activities and enters at least one note for each client into the Echo Advantage System bi-weekly. Communicate on a bi-weekly basis with the client’s Probation Officer. Maintain a receipt book for all restitution payments. Makes timely payments of all restitution to the Program Specialist at Work Bridge or the Clerk’s Office at Berks County Juvenile Court. Engage with clients prior to release from residential program to complete intake/interview process as needed.
