Case Manager Location:

AbraxasWorkBridge, Pittsburgh, PA 15222 Abraxas WorkBridge is hiring aCase Manager to provide opportunities to youth involved withAllegheny County Juvenile Probation to complete court orderedcommunity service hours through an established community servicesite. The Case Manager maintains a client case load from intake todischarge. Responsibilities include finding an appropriatenonprofit community service site to complete court orderedcommunity service hours, conducting placement interviews with sitesupervisor and youth/family, and establishing a schedule conducivefor youth to be successful in completing the hours. The CaseManager is responsible for monitoring each youth’s hours throughfollow up and updates from each community service site andreporting progress to the referring Juvenile Probation Officer(JPO). Once obligations are met, the Case Manager is responsiblefor case closure and submission of discharge paperwork. Thisposition requires travel throughout Allegheny County.

High school diploma and minimum of three yearsof related experience in the juvenile justice system; OR Bachelor’sdegree in related field preferred and minimum of one year ofrelated experience in the juvenile justice system. At leasttwenty-one (21) years of age Driver’s License Physical exam thatincludes TB and drug testing. Criminal clearances (State Police,FBI and State Child Abuse Clearances) Satisfactory completion ofbackground screening and applicable pre-employment checks;including but not limited to employment and/or personal referenceand driving records. Ability to work with computers and thenecessary software typically used by the department. Must pass bothfederal and state background checks which screen for crimesincluding, but not limited to drug related crimes, violent crimes,sexual abuse, domestic violence, child abuse, and repeatedmisdemeanors

Case Manager BasicResponsibilities:

Maintain a client caseload with appropriateservice plans. Develop and maintain relationships with communityservice sites. Develop and maintain relationships with assignedJuvenile Probation Department. Orient the client to program norms,rules, and behavioral standards. Collect the YLS assessment fromthe Juvenile Probation Department and implement it into the serviceplan and the service delivery. Build and maintain a positiverelationship with the youth and their family through being caring,consistent and competent. Support clients and their families incompleting any court ordered conditions including restitution,community service, or specific counseling needs and assessment.

Supervise client activities and enters at least one note for eachclient into the Echo Advantage System bi-weekly. Communicate on abi-weekly basis with the client’s Probation Officer. Maintain areceipt book for all restitution payments. Makes timely payments ofall restitution to the Program Specialist at Work Bridge or theClerk’s Office at Berks County Juvenile Court. Engage with clientsprior to release from residential program to completeintake/interview process as needed.
