Education:
Bachelor’s degree (or higher) in Psychology, Social Work, or a related field OR in an unrelated field with 18+ credits in Human Services required.
Experience:
1+ years of experience in substance use disorder treatment required.1+ years of case management experience preferred.
Experience with adults preferred.
Certifications:
Credential through PCB (such as CRS, CPS, or CAAP) or be actively working toward certification (must obtain within 2 years of hire).
Additional Requirements:
Valid driver’s license and reliable transportation required.
Willingness to drive up to 40 miles from Penn Hills to see consumers required.

Position Overview:
As a Community-Based Substance Use Disorder Case Manager, you will play a key role in supporting individuals in recovery.
Your responsibilities will include:
Case Management:
Provide hands-on support and guidance to individuals in recovery, assisting with accessing resources such as housing, medical care, recovery support (e.g., 12-step meetings), and food banks.
Collaboration:
Communicate effectively with Supervisors and Counselors to ensure all client needs are met and that changes in services are communicated in a timely manner.
Support in
Transition:
Help clients with referrals to higher levels of care and ensure necessary documentation, such as ROIs, is handled smoothly.
Motivation & Education:
Encourage clients to engage in therapy services and provide education to those who may be resistant to treatment.
