Drug and Alcohol Case Management Specialist Position Available In Carbon, Pennsylvania

Drug and Alcohol Case Management Specialist Carbon Monroe Pike Drug & Alcohol Commission, Inc. Lehighton, PA 18235 Private not-for-profit Drug & Alcohol program is seeking an experienced D&A Case Management Specialist to work based out of its Lehighton, PA office. Position is new, providing tertiary prevention services by way of case management work with both students and their families. Working primarily in Lehighton and Panther Valley School Districts and out in the local community, along with some in-office work. Candidate must possess minimum of BS/BA in Psychology/Sociology/Human Services with a minimum of 2 years Drug and Alcohol Case Management and/or D&A Prevention work experience. Prior prevention and/or case management work within a school system a plus. Position requires the ability to relate to children and adolescents as well as their families. Working knowledge of local available resources, social service agencies and communication/computer skills a must. Valid driver’s license and access to automobile required. Position provides specialized tertiary level prevention services within Lehighton and Panther Valley School Districts and the local community, providing a new type of prevention case management to high-risk students and their families who may be referred by guidance counselors, SAP teams, other social service agencies and/or as self-referrals. Position works with families and brokers the necessary available resources to help with barriers to the student and their family’s success, meeting with family members off-site at their appointments, other agencies, schools, public places or in-office as needed to assist navigating those resources and to provide support ensuring successful outcomes. This new program focuses around social determinants of health, including domains of neighborhood/built environment , economic stability, healthcare access/quality, education access/quality and social and community context. Position is F/T, 40hrs per week, typical schedule of Mon/Wed/Fri 8-4:30 and Tues/Thurs 11:30-8:00pm with starting hourly wage of $24.89.
