Aging Care Manager III/Protective Services Columbia Montour Aging Office, Inc. 702 Sawmill Rd, Bloomsburg, PA 17815 Provide casework services to Older Adults to help them attain a more satisfactory social, economic, emotional, or physical adjustment. An important aspect of the work is the employment of casework skills in obtaining essential information needed to aid them in utilizing all available resources. Work is performed in accordance with established regulations, policies, and procedures, but employees are expected to exercise initiative and independent judgment in discharging their duties, seeking advice and help only with the more unusual and difficult cases. Maintains a positive work atmosphere by behaving and communicating in a manner that is understandable and professional with clients, co-workers, supervisors and the public. Completes RON and Protective Services investigation forms in SAMS. Maintains all data fields as required by PDA in SAMS. Investigate the need for protective services, with at least one visit conducted in the older adult’s current environment. Provides protective and supportive services for abused, neglected or exploited older adults. Assist clients who have limited financial resources in budgeting their money and provide information and assistance in applying for financial aid programs pertinent to their particular situation. Using SAMS, writes/enters a complete case file on each consumer receiving ongoing agency services. The information entered may include assessment, care plan, service orders, and ongoing narratives and file attachments. Assists Older Adults in developing and using their own potential for more adequately meeting their social, health, emotional and economic problems. Works in close cooperation with other social agencies, hospitals, clinics, court, and community resources in planning to meet the needs of Older Adults and assists them in utilizing these resources. Complies with documentation standards set by Pennsylvania Department of Aging (PDA) Performs in advisory capacity to other professional staff in specialized areas through participation in conferences and meetings for purposes of diagnosis and plan of treatment. Petitions the court as appropriate under conditions outlined in O.A.P.S. Act. Testifies at Protective Services Hearings. Assist in-home clients with the completion of forms: M.A. applications and Spousal Impoverishment. Do follow-up in cases to see how clients are managing and if their needs have been met. Assist in the completion of guardianship petitions and testify as needed. Provides support to co-protective services investigators to ensure thorough and timely investigations. Provides after hour emergency intervention as part of the Agency’s on call system. Attend State and Regional or other training as required. Maintain an accurate log on mileage traveled and submit for reimbursement as necessary Completes other reasonably related job duties as assigned.
