Bachelor’s degree in sociology, social work, psychology, OR related field; or high school diploma/GED with relevant coursework and two years’ experience in mental health; OR a registered nurse (RN) qualification.
An active and unrestricted driver’s license is required. Reliable transportation is essential.
Strong familiarity with seriously mentally ill individuals preferred.

Schedule:
Monday-Friday, 8:30 am-4:30 pm
Earn $18.57/hour or $20.57/hour with the selection of Enhanced Pay, plus mileage reimbursement
Position Details The adult case manager is a resource coordinator who plays a pivotal role in enhancing the mental health of consumers in Cumberland and Perry Counties. They coordinate services, support families, and tailor care to each consumer’s unique situation. Their work focuses on harnessing individual strengths, connecting with community resources, and promoting stability. While operating independently, they are backed by a dedicated team, all within a dynamic community program. The resource coordinator actively engages with consumers in the home or community setting. Duties and Responsibilities
Coordinate and monitor all supportive resources.
Assist consumers in accessing mental health services.
Assess individuals’ strengths and needs.
Facilitate access to resources that enhance stability.
Provide assessment, service delivery, and problem resolution.
