Job Description

Case Manager–Permanent Supportive Housing Scranton/Carbondale Catholic Social Services of the Diocese of Scranton Carbondale, PA 18407

SUMMARY:

The individual will possess a demonstrated commitment to working with homeless individuals/families and providing social services for client to prevent them from becoming homeless again. The PSHP Case Manager will follow up with clients on referrals made to improve their situation. He/She will be familiar with the local social services delivery system, local Shelter for homeless individuals, local Housing Authority and local city and county officials who are dealing with the homeless of the area. The position may require non-traditional hours of employment at times. The PSHP Case Manager is a full-time position for forty (40) hours per week, as scheduled by the Housing Director. Hourly Rate commensurate with experience.

REQUIREMENTS

Experience in counseling, conducting needs assessment and working with homeless individuals/families and/or those with disabilities, Bachelors degree in social work or related field preferred. Driver’s license needed. The individual must be of good moral character, dependable and reliable. The individual must possess a current/valid driver’s license and insurance and must have access to an automobile while on duty. The individual must consent to a Pennsylvania Access to Criminal History (PATCH) background check and must pass a Pennsylvania Child Abuse History Clearance and FBI Clearance.

TRAINING REQUIREMENTS

The Diocese of Scranton requires that all employees complete an initial VIRTUS Training Program, and subsequent monthly trainings, designed to increase awareness of child abuse/neglect. Employees must also complete all other training programs, out-services, in-services, conferences, seminars, conventions, etc., as assigned by their Supervisor.

CASE MANAGER RESPONSIBILITIES

Provide weekly/bi-weekly case management for homeless individuals in Permanent Supportive Program. Maintain an individual case file for each client and be accountable for all record keeping, bookkeeping and completing of program reports. Ensure all new PSHP clients are properly screened Evaluate individuals’ needs and make appropriate referrals to the local human services agencies. Enter all client information on the Homeless Management Information System (HMIS) Provide assistance in securing housing for clients, engaging landlords and securing apartments for the program. Be aware of the area’s homeless shelters to provide temporary housing for clients until permanent housing can be secured. Become familiar with PA State Department of Public Assistance eligibility and benefits under Welfare Program. Develop an individualized service plan and maintain progress notes on each client. Conduct all interviews of potential PSHP clients and review with housing director. Be on-call for emergency situations with PSHP clients. (phone furnished) Manage the daily operations of PSHP and handle any emergency situations while on duty. Check residence for cleanliness and maintenance on a regular basis; document same. Perform any other duties deemed necessary by the Executive Director/Housing Director.
