Reports To:
Heartwood Center Director Overarching Goal This position is responsible for providing shelter/drop-in center services for individuals and families experiencing or at risk of homelessness and/or food insecurity in the Greater Hazleton Area with a focus on individuals impacted by substance use disorders.
Qualifications:
Post-secondary education or equivalent Knowledge of peer-based services, recovery principles, and homeless services. Excellent computer, written, oral and interpersonal skills. Excellent organizational skills with strong attention to detail. A clear understanding of the services provided by New Roots and Willow Foundation.
ESSENTIAL DUTIES
Provide intensive and individualized case management services with a focus on empowering individuals to overcome barriers. Engage with clients impacted by substance use disorders while experiencing homelessness or facing the risk of homelessness. Conduct comprehensive assessments to determine client needs and appropriate services. Develop and update individualized service plans. Monitor client progress and provide ongoing case management to help clients set and achieve personal goals. Provide referrals and coordinate access to housing, medical care, financial assistance, drug and alcohol treatment, peer support, and other support services. Maintain thorough and accurate members records and documentation that adhere to regulations and reporting requirements of funding sources. Develop, work collaboratively, and maintain strong working relationships with partner organizations and other professionals. Provide crisis intervention and support when necessary. Facilitates and/or schedules life skill groups as necessary or requested. Advocate for clients and ensure they receive appropriate treatment and timely services. Collaborate with the Center Director by reporting utilization changes, demographic shifts, presenting needs, and service gaps. Model inclusive, culturally appropriate language and attitudes. Follow the employee handbook, policies, and procedures. Works cooperatively with other staff, volunteers, community service individuals and interns, assisting with guiding duties, program requirement and center guidelines while on shift. Performs other reasonably requested duties as assigned.
