Full Job Description Position Overview:
The Community School General Case Manager is responsible for being an ambassador of GPCA’s mission, values, and commitment as they contribute to partnerships in an array of community settings. With the approach of working towards increasing organizational stability, breaking the cycle of intergenerational poverty, and having a greater impact on generational poverty the GCM’s will assist students and families to ensure household stability. The General CM role is an integral part of the Community Schools’ team as well as a support to students and families. Under the supervision of the Community School CM Supervisor, the General CM will collaborate with school Administrators, students/families, Attendance Case Manager, and Community School Coordinator to promote household stability which in turn will improve student attendance and enhance educational outcomes. The General CM role is a primarily school-based position at assigned school with an understanding that GPCA offices is their home site.
Key Responsibilities:
Represent GPCA’s mission, values, and commitment in all Community settings Adhere to the agencies Policies & Employee Handbook, Community School Program Standards, and professional code of ethics Serve a caseload of 35+ students in education setting or through home visiting Participate in weekly school attendance meetings and collaborate with Community School partners on identifying students with 3 or more unexcused absences Collaborate with the school and Community School partners on referred students prior to or during the SAIP process to identify and remove the barriers to attending school, slow or stop the accrual of absences, ultimately reducing the number of referrals to Regional Truancy Court Collaborate with Community School partners and school to develop and implement monthly family forums Engage all families within 7 business days of referral to service Conduct an intake and create a family service plan that will address contributing factors that has impacted students school attendance Engage with students/families in a collaborative relationship that will enhance the potential for the client’s completion of service with positive outcomes. Maintain an awareness of community resources, facilitate, and monitor student/family access to appropriate resources and services necessary to support and supplement service goals. Accompany family to resource appointments as needed. Conduct a minimum of 2 monthly home visits where the children and caregivers are present. Participate in all necessary interdisciplinary functions and meetings. Document case progress in City Span Database and Attendance Collaborative Worksheet within 7 days of contact or event. Maintain case file as required by quality assurance. Adhere to Agency and regulatory entity guidelines concerning caseload requirements, service delivery, accountability and confidentiality. Adhere to training requirements established by Agency & Community School Program Standards. Actively participate in cross-training of Attendance Case Manager role to provide coverage if needed Complete 20 hours of trainings for the fiscal year as required by CS Program Standards Attend bi-weekly supervision and department meetings. Flexibility in daily schedule to accommodate events on an as needed basis. Adhere to agency attendance policy and communicate PTO usage request to CS Supervisor effectively and timely Document daily activities on Outlook calendar and share calendar with Supervisor Immediately conference any child safety concerns with CS Supervisor for assessment and make report to Child line state reporting agency as needed Complete all other tasks as required by funder or agency.
