Full-Time Position Summary The IRI Case Manager supports participants in either the Pre-Release or Post-Release phase of Uplift’s Integrated Reentry Initiative. This role delivers individualized, trauma-informed services to adults returning from incarceration, using a combination of Intensive Case Management and Strengths-Based Clinical Case Management models. Case Managers work closely with internal staff and a broad network of external partners to remove barriers and facilitate reentry success. This includes structured assessments, benefit navigation, service referrals, and progress monitoring across a defined timeline of engagement . Program Pathways Pre-Release Case Management (Facility-Based) Conduct 1:1 intake and needs assessment prior to program start Administer standard surveys and assessments (e.g., ACE, digital literacy, risk screening) Identify risks and document participant needs in the Trax database Establish initial case plan and short/long-term goals Complete clinical impressions and barrier analysis Coordinate with instructors to monitor participant progress during inside programming Post-Release Case Management (Community-Based) Conduct post-release intake and reassessment in the community Guide participants through benefit applications, housing support, and employment pathways Deliver and update case plan in Trax based on lived experience and new barriers Support access to medical care, transportation, digital literacy, and ongoing workforce programs Maintain consistent engagement and provide risk-informed, adaptive support over one year Coordinate with Neighborhood Resource Center and post-release supervision as needed Document all updates, goals, and progress milestones in Trax Systems Navigation & Coordination Act as the lead connector for all participant services across the continuum Refer participants to appropriate internal and external programs (e.g., mental health, training, ID recovery) Collaborate with Uplift instructors and staff to produce participant “report cards” Facilitate warm handoffs between programs and ensure data continuity between pre
- and post-release Continuous Improvement
- Develop and refine participant-facing materials (e.g., intake forms, goal worksheets, resource guides) to ensure clarity and usability
- Collaborate with program leadership to identify service gaps and propose curriculum or process adjustments
- Participate in regular case review and improvement meetings to inform practice and share lessons learned
- Analyze case data trends to inform program enhancements and outreach strategies
- Support design and iteration of assessment tools, checklists, and internal workflows
- Contribute to post-program debriefs and planning sessions to evolve reentry supports over time
- Document participant feedback and incorporate into resource planning and future programming
- Assist in preparing materials and documentation for funder reports, audits, and partner updates Data, Reporting, and Evaluation Maintain accurate, timely records in Trax and other case management systems Document risk levels, support needs, service delivery, and clinical notes Contribute to evaluation efforts and grant reporting by tracking participant progress Support Uplift planning through insights into unmet needs and community resource gaps Qualifications Bachelor’s degree in Social Work, Psychology, Human Services, Criminal Justice or related field (Master’s preferred) 2+ years of direct case management experience with vulnerable or justice-involved populations Deep knowledge of trauma-informed care, reentry systems, and service coordination Proficiency in electronic case documentation and progress tracking tools Ability to work inside correctional settings and in the field post-release Proficiency in Microsoft 365, Microsoft Excel, Microsoft Word, PowerPoint, etc.
