Job Description
What You’ll Be Doing You will assist the program’s participants in building on their strengths by providing in-home therapeutic support. Participants include children, adults, and families where clients have behavioral, emotional, and case management needs. Hybrid Role, 20% Remote Work. Your Main Priorities While additional responsibilities are likely to arise on the job, you will primarily:
- Provide community and home-based therapy to assist individuals and families in building a safe and independent level of living. Work with individuals and families dealing with psychological, emotional, and social challenges.
- Conduct Clinical Assessments and develop Treatment Plans for clients at the start of treatment, as well as Discharge and Aftercare Plans at the end of treatment.
- Implement and monitor ongoing Treatment Plans for each family served; coordinate all service components within the agency and in the community.
- Document relevant client information in the agency’s Electronic Health Record system.
- Meet weekly or as needed and work closely as a team member with Child & Family EOS Supervisor and Case Managers to ensure high quality care. Skills & Abilities
- Master’s degree in social work or related field required.
- Minimum 6 months experience in human services or similar role preferred,
- License eligible (LCSW, LMHC, LMFT, LICSW, etc.), licensure preferred
- Strong organizational, critical thinking and time-management skills.
- Basic computer skills required; knowledge of Microsoft Office 365.
- Ability to communicate and effectively interact with a diverse population.
- Bilingual in Spanish preferred.
- Collaboration and teamwork.
- Must authorize Child & Family to complete a motor vehicle records check.
- The ideal candidate will be required to meet all background checks and medical clearances required for the position.
Physical Requirements Direct Service Staff:
This position requires time sitting, standing, walking, driving, carrying, and lifting up to 25lbs. This role also requires operating desktop, laptop, and/or cell phone, and communicating with colleagues.
Travel Requirements Site location:
Middletown or Providence Statewide travel required.
