  • 20 hours per week, flexible after school
  • working with a 9-year-old male.
Job Responsibilities:
  • Agree to report to work on time.
  • Agree to carry out assigned duties and responsibilities as explained to me by the Family Supervisor(s) and as written in this agreement and the Service Plan.
  • Agree not to do activities unless they are specified in the Service Plan.
  • Agree to accurately document the hours I work and to file appropriate tax documents.
  • Agree to ensure proper Incident Reporting.
  • Agree to give feedback to the Family Designee.
  • Agree to keep daily progress notes and to give input on the child’s progress toward Service Plan goals and objectives.
  • Agree to attend and participate in on-going training sponsored by the Family Support Agency.
  • Agree to tell the designated Family Supervisor(s) two weeks in advance of when I need time off and to obtain written approval; this time will be set by mutual agreement between the family and me.
  • Agree to call the designated Family Supervisor(s) as much ahead of time as possible if I am sick or unable to get to work on time due to other problems, e.g. car trouble, etc.
  • Understand that I am expected to be reliable.
  • Agree to give the family and the PASS Agency two weeks written notice if I decide to end my employment.
General Qualifications:
  • Be at least 19 years of age, have a high school degree or equivalent, and two (2) years supervised experience working with children with special health care needs OR
  • Have an Associate’s Degree in human services (i.e., psychology, counseling, child development, education, nursing, etc.) OR
  • Be enrolled in not less than six (6) semester hours of relevant undergraduate course work at an accredited college or university OR
  • Have worked with adults with specific health care needs and developmental disabilities for at least three years
  • Successfully pass the BCI and CANTS screenings Employment compliance with the Americans with

The physical demands of this position are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee is regularly required to use hands in order to handle or feel objects, tools and controls; reach with hands or arms, stoop, kneel, talk and hear. The employee is required to stand, walk and sit and may occasionally be required to lift/transfer up to 50/70 lbs. alone or occasionally up to 200 lbs. with the assistance of another staff. Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.
