Job Description
Case Manager
Various counties – Anderson, Rock Hill, Charleston, Greenville, Spartanburg, Columbia, Aiken
Hours:
8:00am 7:00pm
Flexible hours and schedule available
Payment:
per case based on experience. 80 cases/month max
OBJECTIVE
Case Managers assist individuals, families, and participants with assistance and support through community resources and State programs. Case Managers are responsible for ongoing monitoring and coordination, helping individuals, families, and participants develop goals, and identifying needs and services by creating an individualized plan and monitoring for success. Case Managers work to keep the participants in their homes and provide local resources to help those in need of support thrive in their community and are required to meet with the participants face-to-face a minimum of every three (3) months.
QUALIFICATIONS
Social Workers licensed by the state of South Carolina OR Individuals with a bachelors or masters degree in a health or human services such as field from an accredited college or university, who have at least two (2) years of assessment and care planning experience with clients (experience cannot include more than six months of internship).
DUTIES:
Case Managers must check voice mail and respond appropriately at least twice daily, Monday through Friday, excluding state holidays.
Case Managers must adhere to documentation policy.
Case Managers must check and respond to email daily, Monday through Friday.
Travel to Participants home for Initial Visits, Quarterly visits, and Re-evaluation.
All Case Managers who serve under a Medicaid contract must adhere to their scope of case manager.
Attend training, orientations, and policy meetings as necessary.
Document professionally and in a timely manner per policy.
Always maintain professionalism with clients via telephone or in-person.
Assess and establish a person-centered plan for self-pay and Medicaid participants.
Maintain appropriate dress code, uniform, or business attire.
REQUIREMENTS
Up-to-date TB skin test
Office of Inspector General check
(10) year driving record
Background check
Bachelors or masters Transcript
Copy Drivers license.
Proof of US citizenship
Car insurance verification
Proficient in Microsoft
Resume
Copy current professional license if applicable
Must pass entry test and Case management test Study classes, Study guide and paid training provided.
HIPAA Consent completed.
