Case Manager 4.7 4.7 out of 5 stars Georgetown, SC Great Kids & Awesome Adults is a private agency that provides services for children and adults who have disabilities and/or special needs. We are actively seeking a Case Manager for the eastern/coastal areas we serve in South Carolina. Applicants must have excellent communication skills, be organized, have experience with documentation, and most importantly be able to work independently. Case Managers must possess a bachelor’s or graduate degree from an accredited college or university, and at least one (1) year of experience working with children and/or families with disabilities. Responsibilities Handle case assignments, draft service plans, and review case progress Assist clients in achieving wellness and autonomy Develop effective working relations and cooperate through collaboration with many providers, including but not limited to medical/health professionals, therapy providers, social services, and school systems throughout the entire case management process Interact with patients to keep track of their progress and to ensure satisfaction Adhere to all standards as outlined by protocols, rules and regulations Training and pay: Training is a minimum of 3 months which is contingent upon performance during training. The base salary is $36,048 (this is based on 40-hour work week which includes travel time, billable time, and non-billable time). This amount includes a home-office supply stipend paid at $40/month ($480/yr.). One the 3-month training period is complete, all team members are eligible for: Up to $250/month ($3000/yr.) as monthly performance bonuses, [bonuses are set at 10% ($125/month) and 20% ($250/month) over minimum requirement], Up to $250/month ($3000/yr.) as travel reimbursement which is calculated based on tracked mileage through our Trip Log APP. Pay is non-negotiable.
