Full Job Description Job Summary:

The Case Manager, under the direction and supervision of the Facility Director is responsible for all client related services and documentation. The Case Manager is responsible for identifying and meeting the individual needs of assigned clients beginning at admission and concluding with termination. Communicates client’s adjustment to the program and prepares the client for successful transition into the community. The Case Manager must work a 40 hour week during hours that allow access to clients (primarily evening hours) and perform 24 hour on call crisis services as Staff Duty Officer (SDO) as required.

Primary Duties and Responsibilities:

Be knowledgeable and maintain strict adherence to agency policy and procedure, Statement of Work, and ACA Standards. Maintain facility, staff and client accountability and safety issues in the absence of the Facility Director. Understand client components, level system and programs: Conducts and documents orientation for new clients. Performs assessment of client’s behavioral, psychological and psychosocial development to obtain specific treatment goals. Conduct group counseling. Ensure journaling program is being followed and that each resident participates in the program. Complete comprehensive initial program plan with 14 calendar days. Plan will address all of the clients’ needs and includes a timetable for achievement of the goals as well as next plan date. Conduct Program Plan Reviews every 14 calendar days. Participate in Treatment Team Meetings with key staff weekly to discuss client progress. Provide individual and family counseling and document sessions in case note format. Provide assistance in locating employment/housing. Ensure client accountability (movements, pass/ furlough/ home confinement, etc.) Ensure drug/alcohol/abuse/mental health counseling is provided. Arrange mental health assessments and/or medication monitoring as needed. Provide life/family skills training. Maximize use of community resources. Work with USPO and client to develop a Transition Plan before client is discharged to assist in a successful discharge. Comply with SOW procedures involving resident property, electronic devices, contraband, driving and marriage. Know, understand, and strictly follow disciplinary process. Ensure subsistence is collected correctly. Establish a formal release plan. Coordinate accordingly with CCM, USPO and USM. Strictly follow escape procedures. Follow sexual Abuse/Assault Prevention and Intervention Program. Safety of clients, staff and community shall be given highest priority. The use of physical force shall be resorted to only instances of justifiable self-defense for the prevention of loss or damage to property, and to prevent a client from self-inflicted harm. Only the degree of force necessary to control the situation is allowed; excessive force is prohibited. Any instances where use of force is required shall be telephonically reported immediately to the Facility Director and CCM. Complete all required training. Support agency activities

Education and Experience:

Baccalaureate Degree in Social Work or Counseling from an accredited college or university An approved writing sample exemplifying clinical documentation will be required for employment.
