Responsibilities of the

Case Worker II:

Under general supervision provides intensive high-fidelity Wraparound care coordination for families with children and adolescents with complex behavioral health needs; provides Intensive Care Coordination and facilitates team meetings in accordance with high-fidelity Wraparound principles. Adheres to all agency Security, Privacy and HIPAA policies.
Direct supervision with a level of responsibility requiring the use of good judgment and initiative to complete duties. Employee has some independence related to carrying out the everyday activities and duties of the position due to the “frontline” nature of the job. Works directly with youth, families and formal/informal service providers.
Provides intensive Wraparound care coordination to 10 children with complex emotional and behavioral needs and their families to ensure they receive all required formal and informal services from a continuum of services provided by a variety of public/private agencies, independent professionals and natural supports available to the families. Available 24 hours to support families during times of crisis.
Schedules and facilitates monthly Child and Family Team (CFT) meetings as defined by high fidelity Wraparound standards. Completes assessments with each youth/family assigned and completes assessments with youth/family on an ongoing basis to monitor youths’ progress in care as outlined by agency standards. Intercedes and advocates on behalf of youth and families to ensure appropriate, timely and consistent services, including traveling to and participating in IEP meetings, etc.
In accordance with high fidelity Wraparound standards and COC best practice standards, develops a strengths-based needs assessment/family story and family vision for each youth assigned. Monitors individual youth and family progress, CFT member assignments, and ensures the appropriateness and effectiveness of services to address the needs of the youth and family; facilitates the CFT in exploring changes and revisions to the plan of care as needed.
Maintains current records on each youth documenting contacts, service activity and other required information within COC best practice standards and guidelines; manages administrative duties associated with operating an office as needed. Maintains minimum standard of productivity per month as established by COC.
Develops creative approaches to provide necessary services to youth and families; designs and assists in the implementation of individualized services when more traditional services are not available or not appropriate; procures and/or requisitions services for youth within the guidelines of the state procurement code and COC regulations and with the CFT approval.
Minimum and Additional Requirements
A bachelor’s degree and professional experience in human services or social services programs.

Additional Requirements:

Ability to operate a personal computer and use various computer software to perform job functions. Considerable knowledge of family systems theory, interviewing techniques and team building and facilitation skills. Considerable knowledge of the fundamental types of behavioral health disorders in children and adolescents. Ability to express and present plans effectively both orally and in writing. Ability to initiate and maintain effective relationships with youth, families and personnel from various agencies and external advocacy agencies. Position may require overnight travel.
Position may require employee to work evenings and weekends.
Position requires routine driver duties.
Position may be required to report to work during emergency situations.
Position requires non-essential declaration.
Must have a valid driver’s license. Certification of a negative TB test is required prior to employment. Upon hiring, candidate must submit official college transcript or diploma for degree(s) obtained. Must complete all required COC on the job training within specified timelines.
Preferred Qualifications
A bachelor’s degree in a human service or social sciences related field and one year of experience with children with serious emotional or behavioral health challenges. Bilingual applicants are encouraged to apply. Additional Comments
