S/he will provide individual case management and provision of supportive services and is responsible for all functions related to managing, supporting, and facilitating appropriate services to meet the needs of residents. General case manager will be responsible for maintaining and ensuring all expectations are complete regarding maintenance of individual charts and documentation maintained on the Veterans.

Primary Duties and Responsibilities:

Be knowledgeable and maintain strict adherence to agency policy, procedure of

AWS, GPD

Grant requirements and COA Standards. Maintain knowledge and strict adherence to all Veterans’ Administration standards and agency guidelines, policies, and procedures. Maintain relationships/partnerships with the VA and community resources. Maintain case management for all residents with a focus on housing, employment and finances Maintain appropriate involvement in the supervision of each resident’s care. Including, but not limited to assessment, intake and incident report writing. Ensure the development of an initial treatment plan on each resident within 72 hours of admission and review or reformulate plans for every resident for each 90 days of service. Ensure completion of weekly notes for each resident to summarize their overall status and progress in the program. Prepare appropriate reports on residents for partner agencies. Know, understand, and strictly follow disciplinary process. Establish a formal discharge plan for the Veteran at the completion of the program. Coordinate accordingly with the VA weekly and as needed to include but not limited to the GPD VA Liaison, HUD-VASH case manager or any needed resource provided by the VA Follow sexual Abuse/Assault Prevention and Intervention Program. Support agency activities. Act as lead for treatment team and ensure that proper documentation is present Plan and/or facilitate group sessions for the Veterans to address identified needs and ensure compliance with current requirements Attend and/or coordinate with other community agencies to provide access and resources to the veterans Attend community meetings as appropriate to ensure continuity of care for the Veterans Advocate for the rights of Veterans as needed

$42,000.00 – $44,000.00 per year
