Description:

Purpose The purpose of the Bilingual Case Manager role is to assist patients in obtaining necessary resources and services within Rural Medical Services (RMS) or through outside agencies and service providers. This position ensures patient advocacy, care coordination, and access to essential healthcare and social services to improve patient well-being.

Description of Duties:

Adhere to corporate policies and procedures as well as the mission and values of the organization. Coordinate care with healthcare providers and monitor patient stability and independent living. Conduct home and/or school visits to assess patient and family needs and develop action plans. Assist patients with scheduling medical appointments and ensuring follow-through. Advocate for patients with special needs and help them obtain necessary resources. Support community members and healthcare personnel in assessing, planning, and providing needed health and related services. Refer patients with social and emotional challenges to appropriate community agencies. Educate the community on medical outreach programs and their benefits. Prepare reports required by federal, state, and local programs. Accurately document social determinants and enabling services interventions to address patient social risks. Participate in Quality Improvement (QI), Quality Assurance (QA), and Continuous Quality Improvement (CQI) activities as appropriate. Provide interpretation services for providers, nurses, Care Coordinators, and other staff as needed. Perform other related duties as assigned.

Requirements:
Education Required:

High school diploma, vocational high school graduate, or equivalent (GED) required.

Licenses Required:

Must maintain a valid Tennessee driver’s license.

Experience:

1-2 years of experience linking patients with community resources preferred.

Skills, Abilities, and Other Requirements:

Fluency in English and Spanish (written and oral) required. Strong communication skills with the ability to interact professionally with clients, coworkers, and management. Excellent customer service and patient advocacy skills. Strong computer skills and ability to use related technology effectively. Ability to work independently and as part of a team with minimal direction. Critical thinking skills to assess alternative approaches to problems or decisions. Ability to gather data, compile information, and prepare resource materials. Effective interpersonal skills with the ability to handle challenging situations objectively. Strong organizational and multitasking abilities. Conflict resolution skills. Self-motivated, flexible, and dependable. High level of honesty and accountability.

Physical Demands and Work Conditions:

Work is performed in a clinic or office environment. May require travel for home and school visits. May require lifting objects up to 50 lbs., with frequent lifting/carrying of up to 25 lbs. Other physical activities may include pushing, pulling, climbing, stooping, kneeling, reaching, talking, hearing, seeing, and driving.
