Key Responsibilities:

Provide intensive, individualized case management services to individuals experiencing homelessness. Conduct street outreach and field visits in various weather conditions to engage and build trust with clients. Develop comprehensive service plans tailored to each client’s needs, strengths, and goals. Assist clients in accessing shelter, housing programs, healthcare, mental health services, employment, and other critical resources. Advocate for clients within social services systems, housing programs, and with community partners. Monitor client progress through regular check-ins, case notes, and updated assessments. Collaborate with internal teams and external agencies to coordinate holistic care and support. Maintain accurate documentation to ensure compliance with program policies, funding requirements, and confidentiality standards. Empower clients by promoting independence, life skills, and long-term housing stability. Respond with flexibility and compassion to crises and evolving client needs.

Social Services :
Eligibility Interviews:

Conduct interviews to assess client eligibility for various programs, including food, clothing, furniture vouchers, and housing assistance.

Provide Assistance:

Assist with temporary housing arrangements, transportation, utility and rent assistance, and other essential services. Referral to

Community Agencies:

Serve as a liaison to other agencies, making referrals and ensuring clients have access to additional resources.

Housing Assistance:

Assist clients in locating and securing affordable housing, including home visitations and ensuring living spaces are furnished and maintained.

Administrative Responsibilities :
Case Records Management:

Maintain accurate and up-to-date client records and case logs, ensuring data is entered into the Homeless Management Information System (HMIS).

Statistical Reports:

Prepare and submit monthly reports on service statistics and client progress.

Case Presentations:

Present case information to supervisors and funding bodies, ensuring all required documentation and program outcomes are properly articulated.

Financial Recordkeeping:

Ensure that all financial assistance expenditures are properly documented and submitted to the appropriate personnel for processing.

Skills and Qualifications:
Education & Experience:

Bachelor’s degree in Social Work, Behavioral Science, or a related field. At least three years of progressive experience in case management or social services, particularly working with individuals affected by domestic violence or similar issues. Experience in creating individualized recovery plans and accessing community resources. OR an equivalent combination of education and experience.

Certifications:

Valid State Driver’s License and MVR Report.

Knowledge & Skills:

In-depth knowledge of case management principles, social service resources, and community agencies. Ability to create program plans and assess client progress. Strong communication skills, both written and verbal, for effectively engaging with clients and community partners. Ability to maintain accurate records and manage multiple cases at once. Experience with outcome-based client management and data systems, specifically Homeless Management Information System (HMIS).

Physical & Working Conditions:

Some physical effort required, including walking, standing, and lifting up to 25 lbs. Primarily seated duties, but the role involves periodic standing or walking.
