The ideal candidate will be responsible for providing support and guidance to youth from DCS Foster Care navigating various challenges, including mental health issues, addiction, and family dynamics. The Case Manager will work collaboratively with the youth to develop individualized care plans, ensuring they receive the necessary resources and support to achieve their goals. At Guy Care, we serve youth who are diagnosed with developmental, intellectual and/or physical disabilities. We are seeking a Case Manager that has experience working with this population. Responsibilities Assess client needs through interviews and evaluations to create personalized care plans. Provide crisis intervention and management to clients in urgent situations. Facilitate access to community resources, including healthcare, housing, and employment services. Conduct regular follow-ups with clients to monitor progress and adjust care plans as needed. Collaborate with multidisciplinary teams, including healthcare providers and social services, to ensure comprehensive support. Utilize motivational interviewing techniques to empower clients in their decision-making processes. Maintain accurate documentation of client interactions, progress notes, and service plans in compliance with organizational policies. Advocate for clients’ rights and needs within the community and service systems. Skills Strong crisis management skills to effectively handle emergencies and high-pressure situations. Understanding of child welfare principles and practices for working with families. Proficiency in motivational interviewing techniques to foster client engagement. Experience in childcare or related fields is a plus for supporting families with children. Mentoring abilities to guide clients toward personal growth and self-sufficiency. Knowledge of behavioral health issues affecting individuals and families.
