Responsibilities and Duties:

Under the supervision of the Lead Case Manager, the primary responsibilities of the Case Manager are to ensure a high quality of life for children served, by providing professional case management services which includes ensuring and monitoring the “active treatment” component within the program. Case Managers are also responsible to ensure well-trained and motivated Foster Parents by providing training and consultation on the outcomes established by MillarRich and maintaining compliance with various state/federal regulations by directly consulting with all Foster Parents of the program locations to which the Case Manager is assigned.

Specific duties include:

1. Promoting high quality outcomes for children by training, supporting, and teaching Foster Parents to become independent within their responsibilities established by Department of Children’s Services (DCS) regulatory agencies; 2. Completing necessary supervision requirements by conducting regular and unscheduled visits as outlined in DCS visitation guidelines to the homes and within the community and documenting these visits in the MillarRich Children’s Services (MCS) and DCS online database systems; 3. Assessing standards of quality and compliance in the homes by completing Quality Evaluation (QE) tools as assigned; 4. Assisting the Foster Parents in the development and implementation of individualized daily schedules for children served which are based on treatment plans, safety plans and the individual needs of the child served; 5. Developing, training and ensuring implementation of goals for each child in each of the following domains: educational/vocational, emotional/behavioral, independent living, medical health, relationships & communication and social/recreational; 6. Preparing Monthly Summaries which review safety, well-being, permanency and visitation/contacts and ensuring these are entered into the MCS and DCS online database systems;7. Conducting assessments (e.g., Child and Adolescent Needs and Strengths (CANS), Casey Life Skills, Human Rights Committee (HRC) restrictions, etc.) as needed; 8. Checking the documentation entered in the MCS database throughout the week as well as monitoring the data entered by the Foster Parents to ensure compliance of other management and supervision requirements; 9. Ensuring that all homes are aware of whom to contact in an emergency situation if the Case Manager will be unavailable themselves; 10. Ensuring the implementation of MillarRich policies and DCS policies and guidelines outlined in the DCS contract provider manual by providing training and follow-up, as needed, to ensure compliance; 11. Promoting high satisfaction among MillarRich, LLC’s customers (e.g., persons served, guardians, families, Family Service Worker (FSW) or other DCS Personnel, Guardian ad litem , third party advocates, community members, teachers, etc.) by participating in the development and implementation of proactive action plans and communication follow-up in order to address any concerns or suggestions; 12. Actively participating in scheduled team meetings, including Child & Family Team Meetings (CFTMs), Utilization Review calls, Permanency Plan Development Meetings, Foster Care Review Boards, etc. Attending and supporting Foster Parents in Permanency Plan Ratification and other court hearings; 13. Ensuring strong, effective communication and team-work within assigned homes, between Foster Parents, state monitors, and other MillarRich administrative personnel through meetings and other forms of written and verbal communication as necessary; 14. Communicating needed medical information to DCS/Requested Agency as requested and consented; 15. Assisting Foster Parents/Schools in the education of individuals served, as applicable. Actively participating in Individualized Education Plan (IEP) and other specially called education meetings, as applicable; 16. Assisting in supervising visitations when necessary; 17. Maintaining professional development by completing annual training requirements as outlined in DCS policy;18. Other duties may be assigned as circumstances warrant.

Requirements:
Knowledge, Skills, Abilities and Personal Characteristics:

1. Bachelor’s degree from an accredited college with a major in social work or a related discipline is required.2. One to two years of prior experience in a position of similar scope is preferred.3. Knowledge of Microsoft Excel and Word is required.4. Ability to learn new computer programs and perform data entry assignmentswith precision and reliability.5. Attention to detail is mandatory with this position.6. Excellent interpersonal/human relations skills.7. Excellent oral and written communication skills.8. Ability to maintain confidentiality.9. Ability to plan, organize and prioritize events within the office setting.
