Job Summary:
Provide casework duties to adults with substance use disorders or co-occurring disorders; provide other treatment and recovery support services; participate as a member of clinical staff in developing, implementing, and evaluating program services; perform other related duties as required.
Duties:
Assist clients with resources that are relevant to support client recovery, such as obtaining identification, insurance, SNAP/EBT services. Provide program components, information, support, and follow-up services as needed, such as Orientation, and Community Meetings. Connect residents with SRC’s alumni group and meetings, and promote community events within Shelby House 1. Locate primary housing for clients at Shelby House 1. Assist and refer any person living at Shelby House 1 who has a substance abuse disorder and is not familiar with recovery services offered by SRC. Participate in weekly clinical supervision meetings and submit casework material for review. Consult with other community, social, health, and welfare agencies, while maintaining Community Partnerships. Prior Case Management experience, preferably in addiction treatment. Develop and maintain a comprehensive and current database of relevant resources for persons served by the program. Report all caseload admissions, transfers, and discharges, to Admissions Clerk in a timely manner. Provide client referrals for vocational, employment, and other support/resource services as needed to achieve individual treatment plan goals, including but not limited to physician and mental health appointments, sober living options, letters of attendance and residency, and updates to parole/probation officers and other approved concerned persons. Participates in community outreach through visits with identified, target populations and provides addiction education materials as needed. Maintains documentation of casework including but not limited to aftercare plans, discharge summaries, and other related requirements within the time frames of agency standards. Participate in trainings, professional development and affiliations in accord with agency goals and mission. Other duties assigned

REQUIREMENTS
High School Diploma some college preferred Driver’s License with “F” endorsement Understanding of recovery process and treatment (12 Steps) Able to work independently and with a team

Must be able to sit/stand for 8 hours. Must be able to walk / stand for up 50%of shift Walking, bending, light lifting up to 50 lbs.
