The Case Manager will provide case management services to clients in a residential setting. They will be responsible for documenting all actions according to State and Federal requirements.
Duties/Responsibilities:
- Conduct intake evaluations and interviews, prepare and maintain individual case records; including but not limited to Individual Program Plans, Histories, Progress Notes, Discharge Summaries, etc.
- Review files for EPSD&T (physical examination) needs, schedule medical, dental, and other professional appointments as needed; assure that proper documentation of appointments is recorded in the individual’s case file.
- Schedule and/or attend appropriate staffing for residents on the caseload, being prepared to discuss cases and assess treatment needs and procedures.
- Submit progress reports to the Home County Case Manager and the Resource Management Unit as required by the contract.
- Prepare and submit pass requests for residents on the caseload.
- Serve as a Staff Development Supervisor if assigned.
- May deliver educational talks to interested community groups, schools, churches, or clubs with the approval of the Program Director.
- Perform related tasks in cooperation with the Program Director and in accordance with the requirements of the Provider Policy Manual and the Licensure Unit.
- Attend training and seminars as needed and approved to obtain or maintain licensure or to increase skills.
- Follow all applicable policies of Wayne Halfway House, Inc.
- Participate in the PQI program as requested.
- Demonstrate competence in cultural knowledge, awareness, and sensitivity.
- Perform any other job-related task as assigned by the Program Director.

- Ability to prioritize tasks. Education and Experience
- Bachelor’s Degree from an accredited institution with a major in social work or a related field. If the degree is in criminal justice there must be a juvenile justice component.
- Master’s Degree in the social sciences or volunteer, practicum or intern experiences in programs/facilities that work with children and families preferred but not required.
- At least five years experience in a juvenile facility is preferred but not required.
No felony convictions and must pass a drug screen. All employees must pass and maintain a clear background screening to remain eligible for employment.
