2025-2026 Transition Case Manager 3.8 3.8 out of 5 stars Tiptonville, TN 38079 Purpose of Job The primary purpose of this position is to oversee and manage the Transition School to Work (TSW) grant agreement for the Local Education Agency (LEA). This role requires a self-directed individual who coordinates closely with Vocational Rehabilitation (VR) Transition Specialists and the Special Education Supervisor or designated school contact. The individual is responsible for ensuring that TSW Transition Coaches and Workplace Readiness Specialists maintain regular schedules and fulfill their responsibilities in alignment with their respective job descriptions.
