The ideal candidate will be passionate about helping individuals navigate challenging situations and access necessary resources. Duties Provide case management services to clients, including assessment, planning, and coordination of services Conduct regular check-ins with clients to monitor progress and address any barriers to success Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive care for clients Develop individualized care plans tailored to each client’s specific needs Advocate for clients to ensure they receive the support and services they require Maintain accurate and up-to-date case notes and documentation Skills Strong background in social work or related field Experience in addiction counseling, crisis intervention, and child welfare Proficiency in supervising, mentoring, and workforce development Knowledge of Medicare, Section 8 housing, and childcare services Excellent communication and interpersonal skills Ability to work effectively in a fast-paced environment Compassionate and empathetic approach towards clients’ needs
