Therapeutic Case Manager, (Therapeutic Day Treatment Program)

Responsibilities:
Therapeutic Case Manager
Work with team to create a welcoming and safe environment and a sense of belonging for the students
Teach the students social and emotional skills, provide them with opportunities to practice and give them feedback about their skills
Establish connections with families of students, communicate with them regularly and offer a variety of ways for them to participate in their child’s education
Introduce families to resources in the community and support their connections with them
Coordinate with Teacher/Special Educator to plan daily activities and schedule Coordinate with Teacher/Special Educator to identify topics for professional development and to coordinate times, places and presenters
Co-facilitate student meetings and staff meetings
Communicate and coordinate with Behavior Consultant, School Based Clinician, Social Skills Interventionists and other members of the team Provide supervision to the Social Skills Interventionists with Educational Lead
Meet and provide oversight for the service documentation for billing sources
QUALIFICATIONS
Bachelor’s degree in social work, psychology, human services, or related field (Master’s preferred)
Minimum of 3 years of experience with children who have emotional/behavioral challenges desirable.
Experience working with children and families in a therapeutic, school-based or case management capacity is a plus.
Creative and critical thinker, flexible, eager to learn, team oriented.
SALARY & BENEFITS
$52,603 plus for Master’s candidate, depending upon education and experience Education reimbursements and loan repayment program.
Monday – Friday work week.
