THE POSITION
Veterans Outreach Case Manager Are you a military veteran looking to help other veterans? The Veterans Outreach Case Manager will provide direct support to armed forces, veterans and civilians with a focus on the former. The VOCM will effectively provide case management services to all clients, communicate and facilitate relationships with community agencies, and work with local, regional and national organizations to build community based programs that focus on armed forces and veteran needs. Hours are 8:30am – 12:30pm and travel is required between Derby and St. Johnsbury sites. RESPONSIBILITIES & SCOPE
Contacts all AF/Veteran referrals within 24-48 business hours to obtain needs and setup initial appointment. Assists therapists and Med Provider Team in meeting needs of veteran and active-duty military personnel. Provides home visits, outreach and follow up contacts with veterans and military personnel to assist in engaging them in their continuum of care needs. Assists in creating CCBHC partnership, formal contracts and care coordination expectations with Department of Veteran’s Affairs medical centers, independent outpatient clinics, drop-in centers, and other facilities of the Department as defined in section 1801 of title 38, United States Code. Ensures there is integration or coordination between the care of substance use disorders and other mental health conditions for those veterans who experience both and for integration or coordination between care for behavioral health conditions and other components of health care for all veterans. Ensures every veteran seen for behavioral health services is assigned a Principal Behavioral Health Provider.
QUALIFICATIONS
Bachelor’s degree or higher in human services or related field is preferred. Veteran of U.S. military with knowledge, skills and abilities to complete outreach, develop rapport with veteran and active-duty military personnel. 1-2 years of case management experience preferred. Knowledge of the VA health system strongly preferred. Experience working with low income, diverse population including persons affected upon by mental illness, substance use and incarceration is a plus. Experience with and understanding of CCBHC models preferred but not required.
